How Case Management Works
Case managers follow patients from admission to discharge. We work in collaboration with the patient, their family, and the entire health care team to establish an individualized treatment and discharge plan. This may include evaluating options and services that most appropriately meet a patient's health care needs.
Our case managers works closely with your doctor and nursing staff to ensure that you receive the best care to meet your needs. Our case managers also ensure that your transition from acute hospitalization to the next level of care is a smooth one. Our case managers are active members of our patient care staff, and can be seen addressing patient and family needs in every patient care area.
The French Hospital Medical Center Case Management Department is a professional staff of registered nurses and social workers who are proficient in assessing the needs of our patients and their families. Some of our services include:
- Arranging patient and family conferences
- Coordinating appropriate discharge needs
- Identifying community resources
- Participating in crisis intervention
