Patient Portal

Patient portal provides you with online access to your medical information on a convenient and secure site. View your personal health records, clinical summaries, laboratory and imaging results, as well as instructions and education specific to your care.

Safety Is Our Priority

Your safety is our priority. As a Joint Commission accredited facility, we follow the National Patient Safety Goals as a standard of care. During your stay, staff will be verifying your identity, sanitizing their hands, and confirming your medications, in addition to taking other measures to ensure you are provided with safe care. If at any time you or your loved ones have any concerns regarding your care or safety, please contact your nurse.

Preparing For Hospitalization

Please bring only the minimum essentials necessary. If you are anticipating an overnight stay you may bring personal toilet articles and containers for storing health aids (eye glasses, contact lenses, hearing devices, etc.).

Please bring your insurance card and/or insurance billing information and your Advanced Directive. If you do not have an Advanced Directive you can complete this form and bring it with you or request one upon admission.

Pre-Registration

We are pleased to offer online pre-registration for scheduled tests and procedures in the following departments: Radiology, Surgery, and Family Birth Center. Please note: This is not pre-registration for the Emergency Room.

Medical Records

Patients must request their records in writing. The Medical Records Department will verify your identity by photo I.D., signature, or pertinent questions. We do this to protect the privacy and confidentiality of your records.

Our department has 30 days to respond to your request and make your records available to you.
