Accessing your medical records is easy using our online patient center. You can also download, fax or mail in your request for medical records.
Accessing We offer an online patient portal for you to access your medical records. You or a designated family member can view your medical records, download them, or send them to your healthcare provider. Please call us at (209)467-6343 for proxy access or for help enrolling.
Visit St. Joseph's Medical Center records for access to patient records from before November 1, 2017. To access medical records from on or after November 1, 2017, please visit this portal page.
To start your request, simply download, print, complete, and sign the Patient Request Form. Once complete, fax or mail it to the attention of the Health Information Management department. Please be sure to sign the form, unsigned requests cannot be processed.

Your request will be processed and fulfilled within 15 business days from the day it is received. Please allow reasonable time to process your request. We will mail your medical records to the address specified on the authorization form. An invoice may accompany medical records requests.
