• Interests: Community Medicine, Bioethics, Global Health, Integrative Medicine, traveling, photography, ethnic cuisine, music and yoga

• Residency: White Memorial Medical Center in Family Medicine
• He is a Family Physician and Founder of Mosaic Family Care
• Serves as core faculty member.
• He serves on multiple leadership committees at White Memorial Medical Center, East LA College Foundation, and Monterey Park Hospital
• He is a proud husband and father of 7 children and now a proud grandfather. He enjoys sporting events, and above all spending time with family and friends

• Serves as Didactics Director, preceptor outpatient faculty member, Nursing Home Attending
• Interests include care of geriatric patient, preventive care & diabetes management
• She’s received multiple teaching awards for teaching residents as a preceptor and lecturer.
• On her spare time enjoys traveling & good food

• Primary care physician and hospitalist
• Medical School: University of Southern California, Keck School of Medicine
• Residency: White Memorial Medical Center
• Serves as core faculty on the family medicine inpatient service;
• Serves on multiple clinical leadership committees at White Memorial Medical Center and AltaMed Health Services
• Husband and father of three wonderful children. He coaches flag football and enjoys time with his family.

• Medical School: University of California Irvine School of Medicine where she participated in the Program in Medical Education for the Latino Community (PRIME-LC) program
• MPH: University of California Los Angeles Fielding School of Public Health, focusing on health policy and management
• Residency: CHMC – Family Medicine Residency
• Core faculty member.  Her interests include full-spectrum family medicine, addressing healthcare disparities, and advocating for increased health access. 
• For fun, she likes to watch NBA games, attempt CrossFit workouts, and take trips to Disneyland.

Inpatient Director

• Medical School: Ross University School of Medicine 
• Residency: Arrowhead Regional Medical Center in Colton, CA
• Serves as core faculty.  His interests include working in the inpatient and integrating Point of Care Ultrasound into clinical practice. 
• On his spare time, Dr. Hernandez is an avid weightlifter and cook and enjoys spending time with his wife and their three children at Disneyland

• Residency: UCLA Family Medicine
• Fellowship: Santa Clara Valley Medical Center Obstetrics Fellowship
• Serves as core faculty full spectrum practice, including OB/C-sections
• She is dedicated to teaching obstetrics and reproductive health, office procedures, and medical education.
• In her free time, enjoys volleyball, cooking, and running

Clinical Medical Director

• Medical School:  Kirksville College of Osteopathic Medicine
• Residency: California Hospital Family Medicine Residency Program
• Serves as Clinical Medical Director of the Eisner Health Family Medicine Center, an FQHC & continuity site for CHMC-FMRP
• She’s boarded in Family Medicine, Adolescent Medicine and majority of her medical career in graduate medical education and practice in full-scope medicine with underserved populations
• Free time: Reading, music, exercise and time with her family

Director of Behavioral Science

• Education: Ph.D. in clinical psychology from UCLA
• Provides teaching and supervision in behavioral health and resident wellness
• Multiple journal case study publications on patient encounters in primary care
• Member for the LACPA Ethics Committee and a former ethics member of the California Psychological Association
• He is an avid cyclist, having completed over twenty-one 7-day 550 mile AidsLifeCycle rides from San Francisco to Los Angeles

• Serves as core faculty full spectrum practice. She is dedicated to teaching obstetrics and reproductive health providing care to underserved populations, addressing healthcare disparities, diversity and inclusion in medicine.
• For fun she enjoys hiking, baking, dance, Orange Theory fitness, traveling, volunteering, and collecting plants.
