Admitting & Registration

The California Hospital Admitting Department offers friendly, compassionate service to our patients and their families using our healthcare system.

We want your registration experience to be positive. We encourage you to complete the
Pre-Registration Form so that you save time and are better prepared for your visit to our hospital.

Please note that to effectively process your pre-registration we must receive your completed form at least 48 hours prior to your scheduled procedure. If you have less than 48 hours before the time of your procedure you can pre-register by telephone by calling our Admitting and Registration Office at (213) 742-5580.  

What to Bring

  • Photo
  • ID Insurance card(s)
  • Any forms, paperwork, physician orders, pre-op labs, or x-ray results from your doctor's office
  • List of current medications
  • Your healthcare advanced directive form if you have completed one. (If you do not have a healthcare advanced directive but would like one, the Admitting Department can provide you with information)

Hours of Operation

Monday through Friday
6 am - 6:30 pm

Saturdays
7 am - 6:30 pm

After business hours and on Sundays and holidays, please go to Emergency Room Admitting Desk.

Additional Information

By completing the online pre-registration form, you are submitting your personal information to assist in your registration prior to your arrival at our facility. After you have submitted your online pre-registration form, you will be contacted by phone within two business days.
