Chronic Disease Management

Let St. Bernardine Medical Center assist you in taking control so you can live a happier, healthier life. This chronic disease management program was developed by Stanford University.

This class is for anyone with chronic disease, family or friends of a person with a chronic disease and anyone interested in learning how to live a healthier life. 

Topics include:

  • Healthy eating
  • Problem-solving for positive changes
  • Relaxation techniques
  • Improving communication with physicians, family, and friends
  • Medication 'how tos'
  • Weekly goal setting

Please note: We need 8-16 participants to hold this class.

Classes are available in English and Spanish.
