Our Health Information Management Department facilitates medical record release of information for all patient care areas within the hospital. Our staff is dedicated to delivering high-quality services before, during, and after your stay.
View Your Records from Your Computer or Mobile Device:
We offer an online patient portal for you to access your medical records. You or a designated family member (proxy) can view your records, download them, or send them to your health care provider.

Request a Copy of Your Records:
To request a copy of your medical and/or billing records, download and complete the Patient Access Request Form. Once completed, use the applicable Contact Us method to submit your request.

Requesting a Copy of Your Records be released to Someone Else:
To request a copy of your medical and/or billing records to be released to someone else, download and complete the Authorization to Disclose Form. Once completed, use the applicable Contact Us method to submit your request.

Birth Certificates
Birth certificate information for a child less than one year old may be obtained by calling our Birth Clerk at (562) 491-9303. If the child is older than one, you will be referred to the Los Angeles County Hall of Records, Norwalk, CA which may be reached by calling (562) 462-2137. Or you may go online and order a certified copy of the birth certificate.
