To obtain your medical records, please contact the the Health Information Department (Medical Records) at 562.491.9941 or in-house, ext. 3941.
You may download this form in English or this forma en Español
Fill out the form and fax it back to 562.491.7965 with a copy of your valid driver's license or government-issued identification to the Medical Records Department, located on the 2nd floor of the main hospital. Fees will be applied for medical record copies for personal use. Please allow 5-7 business days for requests to be completed.

Birth certificate information for a child less than one year old may be obtained by calling our Birth Clerk at 562.491.9303 If the child is older than one, you will be referred to the Los Angeles County Hall of Records, Norwalk, CA which may be reached by calling 562.462.2137. Or you may go online and order a certified copy of the birth certificate.
