    The Connected Care California Concierge is a one-stop resource to help you find a doctor who’s right for you and your family, answer questions about benefits and claims, verify eligibility, process ID card requests, and more. We want to remind you to stay safe while having fun in the sun. If the summer fun is interrupted due to an accident, we'll be here. Know where to go in the event of an accident or emergency:

  • Severe allergic reactions
  • Animal or bug bites
  • Constant vomiting
  • Continuous bleeding
  • Shortness of breath
  • Severe head injuries

Dignity Health is distributing and administering COVID-19 vaccines and boosters. At this time, we are offering the vaccine to eligible patients, now available for ages 5+. To schedule your appointment, visit MyTurn.

How to get free at-home tests:

  • Federal government
    With the help of the United States Postal Service (USPS), households can request a total of four (4) tests to be delivered for free. To learn more and order at-home test kits, visit
    Combined, between the retail pharmacy and medical plan, there is an eight (8) test limit per 30 day period per each covered person.
  • Retail pharmacy
    Your plan will cover qualifying COVID-19 test kits through your pharmacy benefit manager, Express Scripts. Purchase your at-home test kits at the pharmacy counter and ask for a pharmacy receipt. Register for an account (or log in) on the site to obtain instructions on how to submit a reimbursement claim
  • Medical plan
    The medical plan will cover qualifying COVID-19 test kits as well. Please use the claim form and include your receipt. To avoid processing delays, be sure to complete the claim form in its entirety. You can mail the information or login to your member portal and follow the instructions for uploading your claim.
