Our Medical Records Department provides support to all patient care areas within the hospital and our urgent care facilities. The staff is dedicated to making a positive contribution to the consistent high quality care provided to you before, during and after your stay at St. Bernardine Medical Center.

Medical Records is open from 8:00 a.m. to 4:00 p.m., Monday through Friday. For more information, please call (909) 881-4358.

We Offer Three Options For Obtaining Medical Records

  • Patient Portal

    Please be sure you have requested an invitation before accessing the Patient Portal. You can request an invitation by calling (909) 881-4358 or emailing [email protected].

  • Fax, Email, or Mail

    Step 1 
    Download, print and complete the authorization form.

    The authorization form must be signed and dated. In order to verify your identification and validate your authorization, we require a legible copy of a valid photo I.D.

     

    Step 2
    Send the filled, signed, and dated form to the hospital via one of the following methods.

     

    1) Fax the form to (909) 881-4547

    2) Email the form to [email protected]

    3) Mail the form to the Medical Records Department.
