Our Medical Records Department provides support to all patient care areas within the hospital, our urgent care facilities and laboratory.
Medical Records is open from 8 a.m. to 4:30 p.m., Monday through Friday. Completed authorization forms can be faxed to (602) 294-5726 or email [email protected].
